Accessing Your Medical Records

The Medical Records Department maintains all medical records related to the care you have received at the Pantai Jerudong Specialist Centre (PJSC).

Accessing Your Medical Records

Access to medical records is a patient’s right as stipulated in PJSC’s Patient Charter and governed by the PJSC’s Release of Information Policy.

PJSC takes the privacy of medical information seriously.  Copies of medical records may be released upon receipt of the following:

For Patient:

ㅤ1. Application for Release of Medical Information form
ㅤ2. A copy of Patient’s ID – National Registration Identity Card (NRIC) or Passport

For 3rd Party on behalf of the Patient:

ㅤ1. Application for Release of Medical Information form
ㅤ2. Authorization for Release of Medical Information form
ㅤ3. A copy of Patient’s ID – NRIC or Passport
ㅤ4. A copy of Requestor’s ID – NRIC or Passport

The authorization form must be completed in its entirety or we will be unable to release your records.

How To Make A Request

You may make a ‘Release of Information’ request by utilizing one of the following methods:

  • Via the ‘Application for Release of Medical Infromation’ Form
  • Via email: roi.info@pjscbrunei.com
  • Stopping by at the Release of Information (ROI) Counter

Kindly submit the filled form along with the copy of patient’s ID to our email address: roi.info@pjscbrunei.com, or drop it off at the ROI Counter, Level 2, Pantai Jerudong Specialist Centre.

How To Collect the Report

When the report is ready, you will be contacted via email or by phone call. Report will be release based on your preference as below:

  • Onsite collection at the ROI Counter, Level 2, Pantai Jerudong Specialist Centre
  • Scanned copy sent to requester’s email
  • Original hardcopy via Courier Services

Patient’s Request

As our patient, you may request a copy of the information contained in your medical records. However, an additional subsequent request is subject to a fee depending on the format of the report. Further information on charges, including incidentals such as delivery and bank charges, can be obtained by contacting the Medical Records Department directly at +673 261 3333 ext. 2189 or +673 737 1763.

The release of information applications is generally processed within six (6) weeks from the receipt of a valid application. However, there are cases where the requests may take longer than the given period. The delay may be due to the unavailability of doctors.

3rd Party on Behalf of Patient

Suppose you seek access to someone else’s records (e.g., a relative, Insurance companies, and/or solicitors accessing information for their client). In that case, patients must authorize the release of their medical information and highlight what information and to whom this information can be released.
